Mental Disorders DBQs for VA mental health claims
A Disability Benefits Questionnaire puts symptoms and their effect on work and social functioning into the format VA raters use. Ours is completed after a telemental health psychological assessment, so it reflects the veteran's current presentation as well as the record.
Overview
Evidence written the way VA rates mental health
VA rates most mental health conditions under the General Rating Formula for Mental Disorders in 38 C.F.R. § 4.130, which assigns 0, 10, 30, 50, 70, or 100 percent based on occupational and social impairment. A rating can only be as accurate as the evidence describing that impairment.
The Mental Disorders DBQ records diagnosis, symptoms, frequency, severity, and functional impact in the structure the rating formula expects. When it is completed carefully, a rater does not have to infer the level of impairment from scattered treatment notes.
Our clinical specialists complete the questionnaire after a telemental health psychological assessment and a review of the records you submit. Our former VA rating experts review it for the gaps and inconsistencies that most often lead to an under-rating.

Built for the rating formula
VA examiners see a veteran once, often briefly. A DBQ prepared from a full assessment and the treatment history gives the rater a fuller picture of how symptoms affect daily life and work.
