Aid and Attendance assessments on VA Form 21-2680
Aid and Attendance and Housebound benefits, paid as special monthly compensation or as an increase to VA pension, depend on clear medical evidence of how much help a veteran needs with daily life. VA Form 21-2680 is where that evidence goes.
Overview
Documenting the need for daily help
VA Form 21-2680, Examination for Housebound Status or Permanent Need for Regular Aid and Attendance, asks a clinician to describe the veteran's conditions and their effect on daily activities.
Under 38 C.F.R. § 3.352(a), VA looks at whether a veteran needs regular help with activities such as dressing, bathing, eating, and toileting, or needs care to stay safe from the hazards of daily life. Section 3.351 also covers veterans who are bedridden, in a nursing home because of incapacity, or who meet the vision criteria.
A form that only lists diagnoses invites a denial. Ours connects each condition to the specific daily activities it affects, so the need for help is clear from the form itself.
